Service Description:
Alcohol and other drug treatment services are provided by Moorhouse Wellness. The service supports clients with alcohol and other drug issues and families who have a loved one with addiction issues. Moorhouse Wellness works with clients to achieve their desired outcomes by assessing their situation, offering personalised education, making recommendations, liaising with other health care services, facilitating referral to other services, and tailoring a recovery plan individualised to the client’s needs.
In addition to assessment, Moorhouse Wellness offer ongoing coaching support to assist their client’s and families of those with Alcohol or other Drug issues.
Moorhouse Wellness do not prescribe medication during any stage of the client journey.
Moorhouse Wellness provide alcohol and other drug education and support to industry and employers. Services of this nature are tailored to employers needs through consultation with employment occupation health and safety teams, health and wellness teams, Employee Assistance Programs or management. Pricing and payment for these services are negotiated separate from the client and family pathway booking system within the Moorhouse website.
Confidentiality:
Moorhouse Wellness is a confidential service. Information that is shared is kept private between its clients and Moorhouse Wellness unless authorisation is given by its clients to do so. If there is reasonable belief that a client is at risk of harm to themselves or others, relevant information would be released as a duty of care to emergency services and/or family, for the purposes of preventing harm. This duty would also extend to the welfare of any children that may be considered at risk.

Consent:
The client acknowledges that participation in service at Moorhouse Wellness is voluntary and agrees to provide informed consent for treatment. Any significant changes to the treatment plan will be discussed with the client, and their consent will be obtained before proceeding.
A Client Agreement form (which includes an Authority to Release and Obtain Information section) is required to be read and signed during the clients first appointment with Moorhouse Wellness or following booking an appointment.
Service Fees:
Moorhouse Wellness has a fee-for-service. Our payment scheduling is available on our website at moorhousewellness.com. The initial assessment/ consultation requires an upfront payment via our website booking system. Supplementary in-person bookings can be made and paid with Stripe, at the appointment and are required to be made on the day. All Discovery calls must be paid upon booking. Please note, cash is not accepted. There are no Medicare, Private Health insurance and NDIS rebates currently available.
Cancellation Policy:
Appointments cancelled at a minimum of 24 hours notice prior to the appointment will be fully refundable or transferrable for upfront payments. Cancellations with less than 24 hours notice will result in a 50% cancellation fee for any upfront or pending payments.
For prepaid appointments booked through the website booking system, refunds will be returned to the card used in payment and will take 5-10 days to appear on the client’s statement. Refunds for payments made via Stripe will be returned to the card used in payment within 72 hours after Moorhouse Wellness have authorised the refund under the above terms.
Responsibilities of the Client:
To review and respond to any communication from Moorhouse Wellness in a timely manner.
Attend scheduled appointments punctually and actively participate in treatment sessions.
Be honest and open with the treatment provider regarding substance use history, mental health concerns, and other relevant information.
Follow the agreed recovery plan and are encouraged to adhere to any recommendations provided by Moorhouse.
Notify Moorhouse Wellness of any changes in circumstances that may impact the treatment process.
Settle any outstanding payments prior to attending their next appointment.
Responsibilities of the Service:
Provide a safe and supportive environment for clients to address their issues.
Deliver evidence-based treatment interventions tailored to the individual needs of each client.
Respect the autonomy and dignity of clients throughout the consultation and treatment process.
Maintain confidentiality in accordance with legal and ethical standards.
Termination of Services:
The Service reserves the right to terminate treatment if the client engages in behaviours that compromise their own safety or the safety of others, disregard the recommendations in the agreed treatment plan, or demonstrate an unwillingness to participate in the therapeutic process.
Complaints Procedure: Clients are encouraged to raise any concerns or complaints they may have about the Service promptly. Complaints will be addressed in a timely and respectful manner.
Governing Law: These terms and conditions shall be governed by and construed in accordance with the state government laws of Western Australia and the federal laws of Australia.
By agreeing to these terms and conditions, the client acknowledges their understanding and acceptance of the obligations and expectations outlined herein. These terms and conditions may be subject to periodic review and updates at the discretion of the Service.
